Hospital Monitoring Discharge Team

The LINk set up a working group in 2008 to improve the process of discharging patients from hospital.  Their work is now being monitored by the hospital discharge monitoring team.

People had told us that they were concerned about the lack of information around hospital discharge and the processes in place.

Members have discussed the concerns with representatives from NHS Norfolk, the Norfolk and Norwich University Hospital, the James Paget University Hospital in Great Yarmouth and the Queen Elizabeth Hospital in King’s Lynn.

All three acute hospitals have been visited to see how the health and social care discharge processes work in practice.

In January 2009 the discharge group undertook a survey about patients' experiences of hospital discharge.  The report went to all the NHS and social care organisations and was endorsed by the Health Overview and Scrutiny Committee and several Norfolk MPs. The discharge monitoring team is now taking forward the LINk suggestions around hospital discharge.
